Please choose your Portage Path provider from the drop-down menu ONLY at the time of your scheduled appointment. If you need to reschedule your appointment, please call 330-253-3100. If you are in crisis, please call 988 or visit Psychiatric Emergency Services at 10 Penfield Avenue, Akron.
